What’s Included Each Month

One Monthly Care Check-In

One scheduled monthly check-in call, up to 45 minutes, to review current care needs, family concerns, upcoming appointments, documents, transportation needs, and next steps.

Appointment Preparation for Up to Two Appointments

Appointment preparation means we help your family get organized before a medical visit, dialysis-related appointment, follow-up visit, housing appointment, or care meeting.

This may include organizing:

Insurance or paperwork questions

Up to Two Caregiver Updates

A monthly care summary is a written overview of what happened during the month, what changed, what still needs attention, and what is coming next.

Basic Document and Task Tracking

We help track care-related paperwork, deadlines, forms needed, discharge instructions, insurance notices, housing documents, appointment paperwork, and family follow-up tasks.

Email/Text Support

Families may send non-emergency care updates, questions, or document-related notes by email or text. Responses are provided within 1–2 business days.

For families with insurance forms, discharge papers, senior housing applications, medical records, family records, appointment paperwork, or care documents that need to be sorted and organized.

Caregiver Family Meeting Prep

Support preparing for a family care meeting, including discussion points, care concerns, decisions needed, caregiver roles, and next steps.

Senior Housing Transition Support

Help organizing the moving pieces related to senior housing, assisted living questions, housing documents, care needs, family communication, and transition planning.

Appointment Follow-Up Support
